A 1987 operative specimen from a 67 year old man who presented with a two year history of a progressively enlarging painless swelling of the right calf. There was no previous history of infection or trauma. At operation the right calf contained a large aneurysm originating from a branch of the posterior tibial artery. The bisected aneurysm shows the greater part of its lumen filled with laminated thrombus. There is a residual lumen at superior pole of the specimen. Microscopy showed the wall to be densely fibrous and devoid of arterial elements. The appearances are those of a false aneurysm of long standing.
