How do AIDS patients know when to stop taking anti-HIV medicines ?I have heard it has something to do with blood test.

From what I know (from friends who have a more direct and personal experience than myself), once you're on, you do not come off the drugs.

If you come off the drugs, and the virus takes hold again (as it almost certainly will) then it will have a strong likelihood of having developed a resistance to the drugs regime you were on.
