Title: Can someone explain a few terms in regards to my osteoarthritis?

My femoral neck is in tact.??

The acetabulum is unremarkable except that there is a bilateral juxtaarticular cystic and sclerotic change in the superior acetabular rim.???

Moderate to severe osteoarthritis of both hips with changes predominantly in the superior acetabular rim with juxtaaticular sclerosis and lucency adjacent to the right and left hip joints.No Acute Fracture.

So could someone explain those details a bit better for me?

She has suspended any lifting or carrying weights or such..My exercise on flat ground has been suspended and the Doctor grounded me to pool exercise along with bed exercise and chair things also..Low impact gentle stuff. I am Trying to get, six to seven days a week in the pool to keep mobility and trying to strengthen muscles..I cannot physically do 6 days in the pool not to mention driving to town every day of the week its too much

added 500 mg glucosimine 3 times a day and zyflamend also.

I cannot have surgery but she says we can work on building the hip area to try to get me in a position to be able to do so.. also taking more weight off.

Any Radiologists in here....? 

I am having no luck figuring out these terms.. some seem to indicate different things..

I have my x-rays here at the house....

Some terms explained:

Femur: the big bone between your hip and knee
neck (bones): the left arrow on the diagram (right arrow is head), and generally considered as the weakest part of a bone.

acetabulum: the cavity in your hip bone where the head of the femur articulates in.
superior acetabulum rim: the edge of the cavity towards your ribs. When in standing position, this rim lays on top of the femoral head and thus transfers the weight of your torso onto your legs
bilateral juxta-articular cystic and sclerotic change: on both sides of your hip, near or in the region of a joint, you have changes in the state of the lubricant fluid (cystic) of the joint and some hardening/stiffening (sclerotic) of cartilage which should be flexible.


I'm no doctor or radiologist, but had some anatomy courses during my education :)

Thanks Nizzle! That helps some...

lucency adjacent, I guess when the x-ray's light hits this part of the bone it is sort of translucent, meaning the bone is so thin you can see through it. So that it reflects the light back through it, or something like that..?

So what are bone tumors all about? Can these lucent areas in the bone be caused by tumors...

I an wondering if the hip is so thin and translucent is there hope of rebuilding bone in those areas? Is that plausible enough to help prevent a major break!

When they say no acute fracture does that refer to the fact that the bone has some holes but has not completely fractured yet or what?

Bone translucency means indeed that there's less dense bone or no bone at all at that place. It does not say anything about the cause (can be osteoarthritis, can be a tumor as well). Whether or not it's beyond rebuilding capabilities is not for me to say because I'm not qualified to interpret X-rays like a radiologist. The blacker the spots are on the X-ray, the worse the translucency and the worse the condition of your bones.

"No acute fracture" is not a prediction, it's an observation. (Acute fracture is like an accidental break, with a one time cause. Non-acute fractures are stress fractures, with a repeating cause, IE an athlete can have stress fractions in his feet from overdoing it on trainings etc)

The x-ray of the bone looks like is very black alg both sides of the pelvis but mostly,holes on the pelvic area where the joint sits and in the Upper hip area above the joints. and o the right side of the right, side of the dog Bone head.. LOL.....

Sorry not up to a detailed response but do bear in mind the nature of radiology reports.  They put down what they see and expertise varies.  Also all has to be taken in context.  If you have this reading and are 20 eg. is more concerning than if you are 70.  ALWAYS get your consultant to review the images, they know better what they are looking for, and know your history better.  And no matter how much you are fobbed off, get more than one opinion, especially if surgery is recommended.

Well I am 48 for 2 more weeks! My hip at this point renders me down for nthe count after very little exercise. It is constant chronic pain even at rest it hurts and aches, It has gotten to the point that my lower back and hip will hurt like crazy shortly after falling asleep in one position for more then about 30 to 45 minutes.
I cannot sit on my bed with right leg hanging off because the pain shoots off the scale.......
Thanks for the advice my second opinion will be on the 27th down at Stanford Medical again..leaving Wednesday night, returning hopefully by Saturday,
Rechecking heart at cardiac center and seeing the rheumatologist again, for systemic sclerosis 2nd opinion,  and the hip and a few other problems.. Friday if possible.

Thanks for the cardiac support btw.  Although from your scan it is likely your hips are causing the problems also be aware of referred pain.  Was my initiating problem, debilitating pain and weakness in my hips and upper legs (one lower leg too now following my car accident - sigh).  Am fully dependant on nearly every major and minor narcotic and benzo going - pill cocktails on the hour it seems now -  and about to run out.  12 years on some of them, take doses high enough to kill most people as have such tolerance.  Feel my life will end if and when I do run out, withdrawal then pain?  Not for me...off the point. Pologies.   Major issue right now.  Am younger than you but after years in a hip replacemet queue was worked out my hips were fine and its my spine that is out of whack - referred pain.  Some may have read my opinions on lazy medicine, so won't start ranting again.  Ironic thing is since leaving the evil profession yearn to go back, behind all the bull really got a buzz out of helping people and putting things right from the inside.  Brand new career now, one where my disabilities don't count.  Other than if I lose my marbles....

You are right about deferred pain and I have alot of that from my shoulder rotator cuff injury...  I know there is also the same in the hip area... It is really intolerable at times especially when I try to go without pain meds... I have them down to three a day but it is just too much and then I start getting bleed through pain and then I get overwhelmed and its harder to get it back to a tolerable place... and still move around.
It sounds like you have your hands full and its vital to try to stay busy...productive.. I have hated loosing my work the kids the activity the fun..etc..I don't have fun anymore... I think the depression really has a lot to do with it so I am working hard to try to get my bearings back and try to enjoy myself!
 I am sorry that you have been handed such a mess to have to deal with at this time...I don't remember reading about an car accident..I am sorry.. I probably did and it has slipped away from me like many things!

You will get through it just hold tight and stay strong of mind.. You can make it .. you just need to believe you can!
