Embarrassing moments are always fun to hear and compare. So spread yours.
Mines the time in the 3rd grade i was impressing everyone by jumping to the 5th bar on the monkey bars so i tried the 6th i smacked my head and was sent to the nurse. That same day i had a party once again i wanted to show off and was diving into the pool and...(you can all guess) my swim trunks came off.

It's interesting that humans should experience certain kinds of miscalculations as painful than others, even though they result in no immediate damage. They are often social errors - showing up dressed inappropriately, failure to acknowledge some significant event or change in another person's life, demonstrating ones lack of skill or ability in a group, etc.
There would seem to be an optimal degree of painful embarrassment that keeps people from being oblivious to other's perception of their competence, but also does not completetly interfere with acting at all or being around people who might witness their errors and miscalculations.
