how does a throwing knife get from the thrower to the target without the handle hitting the target

The blade is heavier than the handle. That causes it to go point-first through the air no matter whether you hold the handle or the blade when you throw it.

A lot of people think that the knife goes end-over-end through the air. Holding the knife by the blade when throwing it reinforces that idea as it has to revolve through 180^o to get point first (it may spin through 540^o once, but that's all).

Accuracy takes practice. But you should be able to get it to go the right way.
