In recorded history, I would guess Tambora (1815 Indonesia) and Krakatoa (Indonesia) had the highest fatalities.  Both generated large tsunamis, Tambora also produced pyroclastic flows in populated areas.  Tambora also caused global famines and climate changes (1816 is known as the year without summer in much of Europe).
Toba (Indonesia, around 75000 years ago), probably gets the nod for non-recorded history.
The deadliest effects of volcanos are tsunamis, mudflows and pyroclastic flows (hot ash).
