What are the bioethical implications of prolonged psychiatric therapy?

Should not doctors evaluate the safety of long-term antipsychotic administration over the subjective benefits it may provides to the patient?

I think they already do evaluate the safety of drugs (including antipsychotics) on a continuous and ongoing basis.
