A volcano is formed when igneous material (molten, also called magma or lava) erupts at the surface of the earth.  Obviously this requires that molten rock be present in shallow levels of the crust- which requires a large heat source.  Almost all volcanoes are located either along plate boundaries or are underlain by hot-spots.
