I've applied the same principle to medical equipment and soon will apply it to a car. You pay for a guaranteed uptime maintenance contract and the contractor refunds part of the fee for every hour that the machine doesn't work.

The NHS capitation fee actually embodies the Chinese principle: your GP loses his fee if you die.It's somewhat diluted but still underpins the tax-funded NHS: every penny spent on prevention should save more than a penny on treatment.
