I read somewhere that the ancient Chinses paid their doctors when they were well, and stopped paying if they got ill.

I've applied the same principle to medical equipment and soon will apply it to a car. You pay for a guaranteed uptime maintenance contract and the contractor refunds part of the fee for every hour that the machine doesn't work.
The NHS capitation fee actually embodies the Chinese principle: your GP loses his fee if you die.It's somewhat diluted but still underpins the tax-funded NHS: every penny spent on prevention should save more than a penny on treatment.
