i want do research in ear nose and throat. i am an ent graduate at present in uk. how to apply for research or a phd programm.can anyone guide me.

basically keep a look out for adverts and apply! you can also try approaching principle investigators in the UK directly and asking them if they have any openings for a PhD studentship in their lab.
I'm assuming you wan to stay in the UK. also I think you might be too late for an october start (which is when most PhD's start) but occasionally you'll find one to start in january or april.
i don't know if there are alternative routes for medical students, so you should make your way to your university's careers centre and find out.
