Not that it's not perfectly wonderful now, and I certainly don't mean to sound impatient but, I'm just wondering how long after I turn 40 does it get better? Is it immediate? Does it take a day or so, or even months? Hubby is younger by 4 months. I wonder if it only gets better when both parties are 40. Hmmm. This is on my mind entirely too much, I think I need a hobby.

There are two kinds of sudden severe headache that are very similar. One is the orgasmic headache and the other is the exercise provoked headache. There is debate on whether the basic mechanism is neurogenic or vasogenic. I think that it incorporates both neurogenic triggering and then secondary vascular dilatation.

These tend to be repetitive and benign. So if one has had seven or eight orgasmic and/or exercise headache it is unlikely something more threatening. That threat is the sentinel bleed, a small haemorrhage due to leakage from an aneurysm or Arteriovenous Malformation. The dilemma is what to do when it is the first time for the headache. Dr. Neil Raskin of UCSF in San Francisco is a world expert on Migraine and Headaches in general. He once told me at a meeting that the first time orgasm or exercise headache must be evaluated (Magnetic Resonance Angiography, catheter Angiography, or spiral CT scan.) The odds are not high but the incidence of aneurysms is quite high in autopsied patients.

If one has several such headaches it is most likely the strange orgasm or exertional headache. There seems to be some prostaglandin mechanism but the trigger maybe related to some sudden flux of noradrenalin and/or Dopamine. But all of these headaches are classified by Dr. Raskin as "Indomethacin responsive headaches."

They are well treated by the non-steroidal anti-inflammatory drug, Indomethacin 25 to 50 mg. It works best when taken prophylactically. If you plan a run around the lake then take Indomethacin 30 minutes to an hour before the run. Or take the Indomethacin an hour before anticipated shagging. Why Indomethacin works is still debated but not the fact that it does work. It is not likely an anti-inflammatory mechanism but suspected to be a prostaglandin mechanism of the Indomethacin.

Disclaimer: I would suggest anyone with this kind of headache to confer with his/her doctor before starting Indomethacin because of potential side effects, and be sure that you may not need neuroimaging to rule out an aneurysm. Family history of ruptured intracranial aneurysms would make me advise neuroimaging.

Hey My husband has experienced these so called thunderclap headaches, not every time but once in a while. I often wondered if it was because his blood pressure may have been particularly high on those occasions. He has just found out he has a touch of high blood pressure. HMMMM! I wonder if that could explain them at all.

I just experienced this particular breed of headache for the first time ever a couple days ago, and it scared the bajeezus out of me. I was thinking "Aneurysm!" too, especially since I have a friend who survived one, and the pain/throbbing sounded a lot like what she described. I was doing what I've done for over twenty years, and it surprised me that I'd have a problem now.

I've tried a few times since then, with the same result all but once. I found greater success by slowing down and monitoring my breathing, being sure not to hold my breath. So, I may not be able to have a quickie ever again, unless this goes away in time. My partner suggested it might be stress related, as I've been through a lot of changes since the first of the year (starting and stopping school, major schedule changes at work, and starting to write a very personal and emotionally challenging book).

Normally, when you go to the doctor and say "It hurts when I do this..." they say "Well, stop doing that." But I don't think anyone here is willing to give up on orgasms without a fight, right?

To that end, I did a bit of digging on google, and came up with the following link:

Check out the 2nd question and scroll down for the answer. Pay attention to the other symptoms that can be warning signs of something serious. But it sounds fairly benign, and treatable with anti-migraine medication. Quoted here:

1. early headache, which is usually moderate and short lasting;
2. orgasmic headache, described by a sudden severe burst lasting 15 to 20 minutes;
3. late headache, lasting for hours or days. These sometimes follow orgasmic headaches.

If you experience any other unusual symptoms, such as a stiff neck, see a health care provider for a check-up to rule out more serious disorders. These ailments may manifest themselves as a stroke, brain tumor, or bleeding in or around the brain.

If your health care provider determines that your headache is just a headache, then you may want to take an anti-migraine medication or beta-blocker that constricts blood vessels. If prescribed such a medication, taking one before having sex could help to prevent sexual headaches.---

I had Number 2 on that list, and it did dissipate within 5-10 minutes after it began and I stopped playing, but it took much longer to leave than it did to arrive. My thought is to take it easy, and go slow/breathe evenly, and see if that does the trick, so to speak. I'll see a doctor if that isn't satisfactory.

Anyway, I hope this helps to ease everyone's anxiety. I was grateful to read that it's more than likely not serious.

I have just started having this annoying problem. The headaches happen at exactly the time of orgasm and it feels like my head will explode. The first time it happened the migraine lasted for 3 days and nothing would help.

I came across this discussion whilst browsing for information on these headaches, and have registered so that I can contribute to the discussion.

I have had 3 of these headaches now, [all within the last couple of days] and they are very frightening. The first two, came on very suddenly, at the point of orgasm. The third, this morning, whilst gagging on a bit of phlegm stuck in my throat [I have a bit of a chest infection, i think].

The pain is excrutiating, and is centered directly behind my eyes. If feels like they're being pushed out from behind. On all occasions, i've quickly gobbled down some aspirin, so I don't really know how long they'd last without medication.

Information on the web is a bit worrying - talk of aneurisms and such - so i'll be seeing the doc, later today, if possible.

Oh, and for those who are concerned about age - I'm a 49 year old female, - almost 50 in fact. I've never had these headaches before. I gave up smoking around 8 weeks ago, and am overweight.

I'm off now, to ring the doctors surgery to make an appointment. I'll let you know how i got on.

Well, apart from the icredulous look on my doc's face, when I explained the problem [he's quite young] nothing much to report.

He took my bloodpressure, which was the highest its ever been - but still within 'normal' readings [136/76]. He looked into my eyes - studying the blood vessels at the back- and he looked at my throat, and listened to my chest. He announced that I have a chest infection, and my throat is quite red [its not sore though]. He then prescribed antibiotics for me.

I asked about the risk of aneurism and such like, and he said that was why he'd looked at the back of my eyes, and that if i'd any bleed problems within my brain, he'd be able to see it on the blood vessels there. Since they are normal, then the vessels inside my brain must be normal too. Not entirely sure that this is true - sounds a bit dodgy to me - but hey, he's had the training, not me.

So, i'm on antibiotics. The 'object of my desire' - and original deliverer of the first two headaches, meanwhile, has panicked and announced that we'll be celibate from now on.

Oh joy! Just as i was beginning to enjoy myself, too!!!

Hello. I'm 19, male, and just had my second headache of this sort. This one being worse than the first. It must be a common thought that you're having an aneurism, because I thought the same thing. I had to sit up, hold my temples, and breath slowly. It didn't last much longer than 2 minutes.

My biggest pain was in my temples. This lead me to believe at first it had something to do with allergies and sinuses(I'm on two allergy medications in addition to shots... I have them severely, so it wouldn't have been out of place). The other point that seemed to give me trouble was slightly behind my earslobes, running down the back of my neck, off to the sides (Hope this makes sense... Maybe an inch from my spine on both sides). I got a faint dizziness, and my head got slightly warm for a minute.

I plan on calling my doctor tomorrow to make sure it's nothing severe. I sorta panicked when it happened, so I wanted to get online and see if it happened to anyone else, and if so, what it was. I'll try to keep you updated. Just adding my story in, and thanking those with advice.

Are you still having these headaches ? My husband claims to have that very same headache. It can last anywhere from a couple of hours to the very next day. The thing we are associating it with is his allergies. He began going to an allergist for allergy shots in mid-April. He goes in every week for them and gets one in each arm. The does of these shots gets higher and higher as time goes on. I googled it and it said that it was due to sinus's. We agree that this is the problem but will be seeing a doctor real soon.

Hi, all. I have just read these many posts and was relieved to find I wasn't alone. I had my first thunderclap headache about 3 years ago, thought I was dying (really) and then found out I wasn't dying-- I was only pregnant w/ twins. I didn't have anymore headaches, though, because twins scared hubby away after 15 years and so there was no partner. A few weeks ago I reached the big "O" and it was great-- no headache.
But now they're back:( They start on one side of my head or the other: sharp and pulsating, right about "the moment of truth." Immediately afterward, the pain changes to an intense throbbing w/ loss of the stabbing aspect of the pain. Seems like it shoots from the starting point to the area right behind my eye- on the same side, lately the right side/ right eye. Then, within about a half hour the pain is dull and throbbing, sometimes "cramping" around my eyeball and it lasts anywhere from a few hours to a couple of days.
I am in my mid-thirties, overweight, non-smoker, with no history of high BP. I lift weights 2-3 x per week & never have it w/ that type of exertion. Also I never had an MRI or anything, and am relieved to read that recurrent headaches of this sort are almost always benign. Apparently it's the first-timers you seem to want to worry about, though I guess if you don't drop dead from a massive brain-bleed at that first thunderclap, chances are... you're okay. Comforting, eh?
