Assuming there is any truth behind the sub-headline, the answer is obviously "those who have worked all their lives and paid their taxes for free healthcare in their old age". 

It is arguable that the actual cost of making, fitting and maintaining a small number of dental implants  is less than that of a crown or extracting a bunch of healthy teeth and supplying a reasonable set of dentures - particularly if the patient is a bit fragile and unlikely to outlast the implant.
